Database of support services available for families and professionals working with them. www.act.org.uk CHASE HOSPICE CARE FOR CHILDREN CHASE Hospice Care for Children charity provides practical and emotional care and support to life-limited children, young people and their families in south west London, Surrey and Sussex. Support is provided at home and in the community by specially trained professionals and at children's hospice in Guildford. Support includes respite, palliative and end of life care. www.shootingstarchase.org.uk CHESTNUT TREE HOUSE Hospice for young people aged 0-19 with progressive life-limiting and life-threatening illnesses. Offers respite care, crisis care, day care, end-of-life care and bereavement support. Facilities include multi-sensory room, hydrotherapy pool, computer room, wet and dry play areas, sensory garden, young person’s wing, chapel and bereavement suite, all areas accessible. PALLIATIVE CARE TEAM Provides care and support for people with terminal illness and their families (cancer and non-cancer). Two full time Macmillan Palliative Care nurse specialists work Mon-Fri for palliative patients and their family, from diagnosis to the dying patient. Works with the Community Palliative Care Nurse Specialist and the Community Primary Care Teams, and St Barnabas House, Columbia Drive, Worthing. Hospital support at Worthing Hospital, Lyndhurst Road, Worthing for Worthing and Southlands Hospitals. ST BARNABAS HOUSE The inpatient unit has beds for specialist respite care, complex symptom control and complex end of life care but no long-term residential care. Also operates a day care service. This is for all advanced progressive diseases with specialist needs. Provides services for the Worthing, Littlehampton, Lancing, Shoreham, Southwick, Storrington, and Henfield areas.
